Re-Certification Procedures

1. Please discard any old applications you may have on file. Use only applications with a date of October, 2007 in the lower right-hand corner.

2. Checks are no longer payable to Ohio TEC, but now must be payable to Treasurer, State of Ohio.

3. NSF (Insufficient Funds) will result in the voiding of a license.

4. Applicants are advised to use only blue or black ink to complete an application.

5. Applicants may use their SSN or their Educator State ID # on the application.

6. Additional information has been added to the application regarding the background check under Section H. It basically states that all applicants are required to complete an Ohio criminal background check conducted by the Bureau of Criminal Identification and Investigation (BCII). In addition, any applicant who has not lived in Ohio continuously for the past five years also must complete a federal criminal background check through the Federal Bureau of Investigation (FBI).
